I got a BA/ lift done March 2018. Just a few months ago, I realized that I am developing capsular contracture. Now it's at a grade III.
Answer: Non surgical approaches to Capsular Contracture There are things that can be done to treat capsular contracture short of surgery. Let me begin by saying that only an in-person evaluation by a board certified plastic surgeon can determine what you have and what is reasonable to do. Leukotriene inhibitors such as Singulair and Accolate have been used in an off label fashion manner to treat capsular contracture. The results vary but are best if treatment is started early. Ultrasound devices (Aspen Labs) have shown some improvement as well. How do I feel about this? Depends. I will try the leukotriene inhibitors for three months and have had some success. I am not sold on the devices but they may have some merit. The industry standard is surgery. Sometimes, the use of acellular dermal matrix (off label) in conjuction with total capsulectomy may also be effective but costly. The first step is to see your doctor and get their advice.

There are also operative modalities, (capsulectomy, capsulotomy, change of pocket, placement of ADM, removal). If the non operative modalities fail, the breasts usually do not get softer with time. They usually will harden, and become more odd shaped. Please speak with a Board Certified Plastic Surgeon about all your options. The ASAPS and ASPS websites are good places to start. Massage is also questionable and there are some medication’s that you can have provided to you at the direction of your surgeon based on your findings. The hard part is that we are not always sure why some people develop a capsular contracture and others do not. If you are averse to surgery, then I would investigate some of these other modalities, however, realize they may have minimal results. I hope that you can get the relief you desire. Have a goal directed conversation with your board-certified plastic surgeon and we are happy to help you in any way we can.

Answer: Non-surgical treatment of capsular contractures can be pursued through Aspen Ultrasound and/or the use of Accolate (based on anecdotal evidence). Aspen is attractive but I would only do that if the provider offered some kind of warranty that your goal will be achieved. If not, then what? I am a firm believer that if you offer technological devices, that you should be able to provide your patient with some assurances that it will work. And if it doesn't I offer a complimentary re-do or provide a credit towards some other procedure like an open capsulectomy.

Surgical treatment is the only satisfactory way of dealing with the problem if it exists. Other suggested modalities would be a waste of time and money. Best of luck.
