Hello the lesions looks like condyloma genitalis which is related to HPV. the lesions may be removed by ellman Radiorfrequency or femilift laser which I use in my clinic also. for rejuvenation I would advise profhilo injection or radiuses injection in the whole labia majora and that area. As the inventor and innovator of the wedge technique in 1994, I reconstruct many women who have had labia reductions by other surgeons, both wedge and trimming techniques. You do have a good result from your labiaplasty. I usually eliminate the abnormal skin in the perineum and the area around your lower vaginal opening when I do a labiaplasty, which is the skin you now want removed. This can be done to give you a better aesthetic result with most of that skin removed.

Some skin can be smoothed by trimming it away slightly. Other areas can be smoothed by injecting a small amount of fat underneath. The area in the photo is tricky because it's not good for fat injections and trimming away skin might not be optimal. A live consultation in person with an expert might be best.

I would recommend seeing your surgeon that did the labiaplasty. They could easily repair that area with an episiotomy repair. If you do not want to do surgery, you could try a radio frequency device (like Viveve) which I use to treat patients externally and have great results. Other alternatives are microneedling with PRP (platelet rich plasma). Good luck, and I hope that helps.

What you wish can be done (mostly!). The "bumps" you have are normal gland openings. Many women have this anatomy. They can be partially removed, and the "bunched-up" skin around the vaginal opening stretched out, minimizing the redundant skin, with a mini-perineoplasty. Any skilled female genital plastic/cosmetic surgeon can do this (in New Jersey, you might try Dr. Marco Pelosi or Dr. Ammar Mahmoud). Additionally, any remaining "bumps" might be air-brushed away with laser re-surfacing. I'd suggest a consult with an experienced P.S. or Cosmetic Gynecologist to discuss details further.
