I'm 32 years old and have had sunken/hollow under eyes for years. I'm very self conscious about my eyes, especially the wrinkles and folds that form every time I smile. I'm getting married next month and looking for a quick non-surgical solution. Actresses never seem to have wrinkles under their eyes when they laugh so just wondering if there's a quick fix that I'm not aware of. Will fillers or botox help? And which type of filler and where - tear trough or cheeks?

Hi @wanderlust223! The upper cheek is a major support for the under eyes. So when there is volume loss there, not only do the cheeks reflect this, but the under eye can as well with the development of tear trough and under eye hollows. Addressing the volume deficit in the cheeks with the use of a  volumizing hyaluronic acid filler would be a reasonable place to begin.

The eye area needs significant combinations to get improvement. Lasers like erbium, co2, halo, profractional/contour, or Fraxel can all be used to build collagen long term. Plasma pen is a newer less invasive option but will take many treatments and require proper skincare. Plasma Pen is focused electrical plasma energy to tighten skin, producing small columns of energy just above the skin, allowing the skin to tighten to help improve wrinkles. This area will have 3-5 days of small “dots” that will then scab and peel, then the skin will tighter and brighter after. Hello, and thank you for your question. You can benefit from a combination of Botox and filler treatments. Botox will target the lines around the eye area, while fillers can treat your tear troughs. By injecting small amounts of filler (e.g., Restylane and Belotero) around the tear trough, the skin will become plump and supple, lifting the skin, reducing the discoloration, and smoothing out the visible groove in the skin.

Laser and energy-based treatments, including RF microneedling (Profound RF) that help collagen production can be very effective in refreshing the skin around the eyes too. There is also facial fat transfer to restore volume under your eyes. Cheek fillers are the first step for addressing the issue of infraorbital holllows and tear troughs, since this region is the support area for the whole under eye region and volume deficit in this region is largely responsible for the problem. A robust HA filler, such as Juvederm Voluma, works well for this purpose. Very likely, once the volume is restored, the under eye issues may be substantially improved to the point that no further treatment is necessary. If additional improvement is still desired, then a lighter, coherent filler, such as Belotero Balance, may be used in this region.
