Is it possible to soften one's deep set eyes with fat grafting (or other similar procedure)?

Without photos and clinical evaluation it would be difficult to give you good advice. Generally speaking, it is possible to place fat grafts deep underneath the upper eyelid muscle and along upper rim of the orbit to improve (soften) deep set eyes. However, without good clinical examination it is impossible to say whether it is applicable in your case.You should seek a consultation with an experienced board certified plastic or oculoplastic surgeon in your area for detailed evaluation. Fat grafting can be done to correct hollowing of the upper eyelid.  Please seek a board certified plastic surgeon who is experienced with fat grafting.  In this area, the procedure needs to be done very carefully to avoid irregularities.

Fat grafting can be done to the supratarsal fold of the upper eyelid through either fat injections or dermal-fatr grafts. That can make that portion of the eye look less deep.

In certain cases it may be possible to correct deep set eyes with periorbital fat grafting.  As with any surgical procedure there are risks and benefits from this procedure and you should discuss these issues with your surgeon to see if you are a good candidate.  Best wishes, Dr. Lepore.
