Can revision rhinoplasty correct an under-projected tip? (Photo)

This may not be the answer that you want to hear, but you had some very difficult problems preoperatively (severely twisted middle vault with collapsed  upper lateral cartilages and a saddle nose deformity). While not perfect, we rarely achieve perfection and that result  looks to me markedly improved from preoperatively. While you are now 5 months postop (not 1 month), things will continue to settle and improve for the next 6-12 months.

I do agree that you have decreased tip projection at present, but overall the result is a definite improvement. There are several techniques, including tip grafts, extended spreader grafts, and caudal extension grafts for example, that can increase tip projection and counter rotate the upturned tip. It's best to wait about 1 year before undertaking revision surgery in general. It's important to have advice from an expert rhinoplasty surgeon in this matter.

I understand your concerns but I must say that you have had tremendous improvement with a very difficult nose. Tip projection and a decrease in tip rotation can be achieved with a revision but the question is timing. If the tip is firm and still swollen, which is likely at 5 months,  I would wait until this has resolved so your revision will be more accurate.

One month after surgery is not enough time to judge the results. You will be better to judge at 6 months but wait 12 months if revision is needed. A septal cartilage tip grafting will increase tip projection.
