What are the risks of hump removal, bone rasping, osteotomies and other techniques of reduction rhinoplasty?

What are some of the risks of hump removal, bone rasping, osteotomies to narrow nasal bones, or other techniques in revision revision rhinoplasty? So far it seems they include nasal valve collapse, a bridge caving in, and nasal bones widening. What else? How can the risks be avoided or corrected?

Revision rhinoplasty is one of the hardest operations to perform correctly in the entire field of cosmetic surgery. Even the best revision rhinoplasty surgeons have a 10-20% touch-up rate to deal with minor little imperfections. Valve collapse is prevented by the use of spreader grafts. The bridge does not cave-in unless too much cartilage was removed. The nasal bones are narrowed with osteotomies and he usually they stay in the in the new stabilized inward position. That's why the cast is applied across the nose and left in place for one week. Remember, bone and cartilage are living tissue and they can respond to surgical manipulation. On top of the living bone and cartilage, as the soft tissue envelope which shrink wraps across the new bony and cartilaginous framework. In addition to that, it takes a full year for all the swelling to subside from a rhinoplasty procedure.

Risks of these techniques in revision rhinoplasty are similar to those in primary rhinoplasty.  You are correct in that some of the risks include collapse of the bony vault of the nose, collapse or deformity of the bridge, and widening of the nasal bones.  Other risks can include asymmetry of the nasal bones, over-narrowing of the nasal bones, and incomplete osteotomes leading to visible deformities.  The risks can effect both the appearance and function of the nose, however, are all relatively rare.These risks are best avoided with good communication with your surgeon before the procedure.  Make sure you two are on the same page when it comes to what you want.  The healing process is also important to avoid any risks or complications with bony work.  Make sure you protect your nose after surgery and leave your dressing in place as directed by your surgeon.  The dressing is very important to keep the new bony work in the correct position as it heals.  After the dressing comes off, be very gentle with your nose for the next few weeks.
