Been a year since my over the muscle BA. I was told I didn’t need a lift but that is obviously very untrue. I feel like the PS recommendation for overs was not correct. They have a weird shape and just hang. I hate the dips in my cleavage and upper pole. My chest is flat and long. What am I looking at to make them look nice? I need to save up and feel completely lost. I think the implants are too small given the about of empty feeling skin. See before, 1 yr after and goal pics.
Answer: Upper pole fullness Thank you for your inquiry. I do think a breast lift would give you more of what you are looking for. When the nipples tend to point downward instead of straight ahead, that is when we recommend a breast lift. Adding implants to sagging breasts will just give you a bigger version of what you started with. A mastopexy will remove the loose skin, bring your nipples up to the center, and give you more upper pole fullness. If you decided on a breast lift, you don't need to change your implants unless you wanted to change the size. I think the current size is good and you would have the results you are looking for with a breast lift. I'm sorry that you are feeling lost and unhappy with your results. Considering that you have enough soft tissue coverage, there is no need to place the implants underneath the muscle, although it is a possiblity. Cleavage and upper pole fulness will be improved with lift and impant lift in a more proximal position but it can be further improved with fat grafting; I would not suggest doing that before lift result have settled (at least one year after lift). Answer: Implants You need a lift. You just choose implants which gave your larger and still sagging breasts. However, your breasts naturally sit somewhat low on your chest and the bases cannot be raised. You will not have high perky breasts even with a lift, but can get much improvement with a lift.

Answer: Everything you said is true What to do: completely remove the implants and their capsules (relatively easy being over the muscle). Create a subpectoral pocket for new implants. Mesh should be used to preserve the new pocket and obliterate old pocket. Perform a breast lift. Go to a breast expert.
