I'm 23, 3 kids im a 40DDD,and want a Breast Lift. What changes can I expect in breast and areola size?

Thank you for your question.  It is impossible to give you a specific answer without photos.  With a cup size of 40DDD it really depends on what your breasts look like, how much they sag, to determine whether it is even worth doing a lift if you are unwilling to allow some reduction.  If your nipples are above the level of your inframammary folds but your breasts appear saggy to you then you may end up very unhappy a year after your lift when your breasts look almost like they did before the lift but now have scars on them that you paid a lot of money for.  That is because the weight of those DDDs combined with gravity is not a good combination.  On the other hand, if your nipples are pointing to the floor and are halfway down to your belly button then a lift will definitely make you look better and although your breasts won't stay up where they are right after surgery, they will still look pretty good compared to what they used to look like.  You should schedule a consultation with an experienced plastic surgeon who is certified by The American Board of Plastic Surgery and preferably also a member of The American Society for Aesthetic Plastic Surgery in order to learn what your options are.

A breast lift may be a reasonable option
just based upon your description, but an exam will be necessary to make a
determination. Find an expert in breast surgery in your

A mastopexy with or without a reduction accomplishes reduction in diameter of the areola, it's repositioned to the normal location , And reshaping of the breast.

No plastic surgeon can determine accurately what size your end result would be.  What can be expected would be much smaller breast with better shape and position, smaller areola and an overall pleasing aesthetic outcome.  Consultation with several board-certified plastic surgeons is recommended.

If your breast are not reduced, you will have saggy breast within a relative short period of time. Everything that goes up must come down. Unless you are willing to reduce the size of your breast and a proportionate areolar reduction, you will not achieve the results you are looking for. Do your homework.Always seek out the opinion of a surgeon certified by the American Board of Plastic Surgery with years of experience in cosmetic breast surgery. Furthermore request to see before and after photos of previous patients by that doctor....not a clinic or surgery center. Do your homework......research and verify the doctor's credentials. Have they had problems with the Board of Medicine, disciplinary or otherwise. Any law suits? How about the center, clinic or facility? Are they accredited by a national organization or do they just have State approval. Understand that at the current time, there are three nationally recognized organizations responsible for the highest levels of patient safety, AAAASF, AAACH and JCHO. You owe it to yourself to position yourself for the best possible results but under the most stringent safety regulations, If you have kids, even more so.How about anesthesia? Will you have a medical doctor certified by the American Board of Anesthesiology or a certified nurse anesthetist (CRNA)? Understand that there is no substitute for research. It is likely that you will need a reduction in volume as well, but without seeing you, it is difficult to say what would be best for you.

If your breasts are a 40 DDD and you may want some of the weight removed at the time of a lift or a breast reduction.  You can keep the areola any size you desire.  

It is difficult to say what the best answer might be for you since I do not see any photos.  Often, women will wear bras at least one cup size too large when their breasts have a significant amount of excess skin.  Once the excess skin is reduced with a breast lift, the cup size reduces even though tissue has not been excised.  That may be the reason for your increase in cup size.  The size of the areola is reduced as part of the mastopexy procedure to a diameter just over 4cm.  All of this will be explained in great detail during your consultation with a board certified plastic surgeon.
