Can I safely have a Mommy Makeover with sickle cell thalassemia and thombotic thrombocytopenic pupura (TTP)?

In my practice, patients with certain medical conditions will need to undergo pre admissions testing to ensure it is safe for surgery. On occasion, a letter of clearance will be requested from the patients doctor.

I would recommend setting up a consultation with two or more experienced board certified plastic surgeons to ensure that it is safe for you to undergo surgery.

Due to your health conditions, your plastic surgeon may require you to acquire additional clearance from your specialists. Both of these diseases (thrombotic thrombocytopenic purpura and sickle cell thalassemia) are complex with multiple types that can affect the safety of surgery. Thrombotic thrombocytopenic purpura in particular is dangerous and does carry a significant chance of death if you develop it. The first step would be to talk with your hematologist since they should know your specific disease type and discuss the risks of surgery with your conditions and how to best reduce that risk.

For elective surgery, it's in your (and your surgeon's) best interest to be as medically optimized as possible. I would suggest meeting with your hematologist in advance, and having them draft a letter explaining your condition(s), and how they would impact your risk for bleeding/clotting during an elective surgery. In addition, any recommendations your hematologist has for your pre/postop care could be included.
This information could be provided to your surgeon at the time of consult, so he/she could make real time recommendations about surgery and safety for you.

You are very prudent in searching for a team of doctors before having any surgical procedure. Get the opinion of a doctor is well versed with your conditions as well as a reputable experienced plastic surgeon. They should confer with each other to make sure that you’re making the safe decision.

every plastic surgeon has his own preoperative protocol which he recommends to his patients. You will need to get a clearance from your doctor prior to surgery. I would suggest you to discuss this with your plastic surgeon.

You will need to first discuss this with your medical doctor and hematologist to see if you are stable enough for the procedure. Liposuction is a lower risk procedure since it is not too invasive.

Sickle Cell anemia clinically differs from person to person. Although there are publications that sickle cell patients had successful events without any complications under optimized circumstances, there is no data of sickle cell plus TTP patients. So this is a complex situation where plastic surgeron must be in contact with the hematologist.
For the plastic surgeon's view: the operation must be kept short, bleeding must be minimized as possible. Liposuction must be kept minimal or none. Fat transfer I would't suggest. But breast reduction and tummy tuck with a minimized dissection will be of help.

Safety is always the number one priority with surgery. You will want to begin by speaking with your hematologist about your interest in having plastic surgery. If given clearance you will then want to speak to a board certified plastic surgeon to determine if you are a candidate for the procedure and if so, what extra precautions are necessary.
