Gynecomastia Surgery was 1 year ago - Kenalog injections not breaking it up. Is this scar tissue/residual gland? (photos)

It is impossible to tell from a photos whether residual fullness of the breast is due to breast tissue, fat or scar tissue. Even an exam is sometimes hard to tell precisely. If kenalog injections do not make a difference it is most likely breast tissue. Hopefully your original surgeon will want to help you achieve the results you both were working on. Good luck.

I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.
