I’m interested in a revision mastopexy. Current implants are sub muscular 335, with natural tissue I’m a 32j/k at 5’9” 145. Pre kids I was 30f. Can I get a significant amount of tissue and current implants removed and go to larger implants, approx 800/1200cc to get the look I want and remain in the 30f range? Also wanting an internal bra, is over or under muscle best? I do workout, weights mostly.
Answer: Implants Please do not choose 800-1200 cc implants for you will stretch and thin your own tissue and you will sag with time. This would be a very poor choice your your frame and tissue.

Answer: Your Goal Photos Are My Patients Good evening, I perform over 250 XL augmentations a year, more than any other surgeon in the U.S.- seems like you have found some of my patients! And of course every patient of mine gets the Original Internal Bra, my strong permanent internal suturing technique- I think this is critically important when getting large implants. I first developed this technique over 20 years ago, and today it is the most common revision procedure I perform, at least 5-6 times a week- it works and it lasts! In terms of pocket choice, when going big you can't change pockets, so overs stay over and unders stay under.

Answer: Breast Revision Thank you for your question. You are an excellent candidate for a procedure I developed termed "Reductive Augmentation" in which we perform implant exchange to an ultra high-profile silicone implant, lateral and inferior capsulorrhaphy-internal bra, and revision mastopexy. I published in 2017 you can view it on my website under publications. Best wishes to you.

Answer: Implant size Dear Fitnessmom2, breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small. You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.
