Indent and muscle grabbing when flex or use pec muscles after revision and implants under muscle. Is this normal? (photo)

Question

2012 implants over muscle. 2014&2015 capslectomy surgeries. 2016 replaced implants under the muscle with stratus. Now have indent in cleavage and under right breast. The skin grabs the muscle and deforms when I flex or use chest muscles. Is this normal? What can I do to make it better? I went from super secure and feeling good about my BA to insecure and regretting my decision. I feel like this many surgeries is abnormal and so costly! Not worth it!

When your implants were placed under the muscle, a portion of the muscle was detached (this is the dual plane method.) The edge of the muscle has now adhered into the scar capsule, which causes the animation deformity. Converting to the split muscle plane will correct the animation but preserve muscle coverage over the upper pole where it is most needed.

Without an in person evaluation, it is very difficult to determine what is causing this indent.  From your description, it sounds like it may be due to the muscle attachment to the overlying tissue causing some animation deformity.  I would recommend discussing your concerns in person with a plastic surgeon who can discuss the different options for revision.

The indentation is common post multiple surgeries and revisions. The best option would be fat grafting to fill the depressed areas. This can be done under local anesthesia.
