I need a revision rhinoplasty after failure of my first rhinoplasty, and one of the issues has to do with removing a hard lump of scar tissue located just under the skin of my columnella....How is this area accessed during a closed revision as opposed to an open revision rhinoplasty? Thank you.

In general it is ideal to wait at least 12 months before any revision surgery to allow full healing to occur. There may be a few exceptions to this, and this would be best determined by your rhinoplasty surgeon and your particular case.
