The first is the minimally invasive route, which is with filler. HA fillers such as juvederm volbella/ultra, or restylane silk/belotero can do wonders to help improve and volumize the upper lip. That being said, you have to be careful as you can also increase anterior projection if you attempt and volumize too much - giving you the dreaded duck lip. Balance between the upper and lower lips is of utmost importance as well. So don't be surprised if you need a little of filler in the lower lip to balance the upper lip at the same time. Your other option is more permanent. Fillers will last you anywhere from 6-9 months. Surgically, you could do a lip lift. The small incision goes under the nose, and a modified lip lift is designed to elevate the lip, give volume, and improve that contour that you are looking for. Basically recreating that exact motion that you are doing on the third picture. You may not get as much volume from the lip lift alone, and a small mucosal advancement can help recreate that fullness you desire. The reason your upper lip looks better with your lifting maneuvers is because it better suits your lower facial proportions. Hyaluronic acid fillers with some lifting capacity can provide this elevation to increase your upper lip volume. Be cautious not to overfill your upper lip though and make it look disproportionate to your lower lip and lower face dimensions. A good starting point is to aim for the central upper lip vertical volume to be 0.6-0.7 of the central lower lip vertical volume.
Thank you for sharing your question and your photos.I think you are answering your own question in your photos! What you are showing is the result of a lip lift. Lip injections can make the lips fuller, but they project in in three dimensions; up and down but also OUT. For some people, this is the effect they want. For others, they do not want full, round lips, they only want the lip to be longer vertically. In this case I would recommend a lip lift. The lip lift also gives a bit more tooth show.I do these procedures in the office with local anesthesia. It takes about 45 minutes. There are stitches that stay in for 5-6 days and potential for a little bruising and swelling in the week after. If the procedure is done well, the scar is well hidden in the shadow under the nose. The lip lift is performed less now due to the popularity of fillers, but it should not be forgotten! It give a different result than filler. Most importantly, it is permanent!
