Context: 37 year old East Asian female, have lost weight previously (body recomp from size 12 to as low as size 6 - approximately 25-30 lbs), active lifestyle, weight lifts 3-4x weekly Even at my smallest (165 lbs, size 6) my legs and arms were always flabby despite my core not being big. Neither seemed very responsive to diet and lifestyle changes, so I lipo’ed my arms and now, I’m ready to do something about my bottom half. What areas would be recommended to streamline the appearance of my legs? I don’t even mind having thicker thighs, I just abhor the fat knees and the thick calves. If I just did knees, would that help the overall appearance? Thanks in advance.

Answer: Lower extremity liposuction I just responded to another one of your posts. I’ll try to keep this more brief. Lower extremity Liposuction is technically difficult and often does not deliver. The results patients had hoped for. Some parts of the thigh respond better than others. Recognize that Liposuction will never make your skin look better. It can make your skin look worse. Liposuction never flabbyness and Liposuction will never treat cellulite. In regards to Liposuction around the knee, the area on the inner side of the knee respond well to Liposuction, but all the other areas around the knee do not respond well to Liposuction. The upper inner thigh is often requested. This area is also difficult to get good improvements with. If too much fat is removed the skin can look awful. Outer thigh, Liposuction can give good results, but in your case will reduce your feminine curves. In a nutshell, we simply don’t have good treatments for women who have flabby thighs if I may use that term. For lower extremities, I would probably pass on Liposuction or probably pass on any treatment for that matter. I know you want them to look better, but we simply don’t have really good treatments for this area and treatments like Liposuction can make things much much worse. If you want to have liposuction of your thighs, you need to be extraordinarily careful with provider selection. You need to see at least 20 previous cases of patients who look like you and had good outcomes. Lower extremities are very easy to leave disfigured in the hands of those who are not experts at Liposuction. Treating your midsection should be straightforward, but I would be cautious when it comes to your thighs. Be careful, Mats Hagstrom MD
