The lady omental angle is there because of the projection of the chin not because of a lack of tissue in the lower part of your lip. When doing aesthetic treatments it’s generally better to treat the primary problem head-on but sometimes that involves more complex procedures. Fat transfer is not a good approach in this case for a multitude of reasons. The area of concern is fairly specific and thin. This does not do well with grafting fat. Fillers on the other hand work very well in small areas where precise treatments are needed and can be adjusted. In order for grafted fat to survive there has to be sufficient host tissue to support the graft. That area is not one that has an abundance of tissue to support any substantial amount of grafted fat and most likely attempts at grafting will either fail or end up looking bulky. Before attempting a permanent solution especially when you’re not treating the primary problem using a temporary option is a good idea. Fillers can be adjusted with each treatment until the volume and placement is ideal. Ones grafted removing previously placed fat can be very difficult if patients don’t like the outcome. Grafting fat is much better for giving overall diffuse volume. Best, Mats Hagstrom MD
