Either saline or silicone implants under the muscle would work fine. With saline there is no chance of misdiagnosing an implant leak. Fat transfer is not an option . You simply don’t have enough fat.

it is hard to tell for sure without an examination. Since you already had breast lift, it looks like you can place new implants in order to restore some volume. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

Typically patients have two options following explant to increase breast fullness. 1. Liposuction with fat transfer to the breasts (requires adequate fat to transfer, may require multiple sessions for significant breast size increase, provides limited upper breast fullness/shape), 2. breast augmentation with implants (one surgery, more control in breast size and shape, provides improved upper breast pole fullness). Either option can provide good results depending on your build and overall aesthetic goals. A more detailed review of your history and exam would be required to determine which options is best for you. I hope this helps.

In my opinion, re-augmentation with an implant is in order. Your nipple position is great but you need an implant to do the rest.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Hello, in your case it is required to fill the tissue with breast implants, I don't see that you have a lot of fat to suggest fat transfer, so the best option is implants.

Thank you for your photographs. I’m sorry about your misdiagnosis causing an unnecessary surgery. However, if you were pleased with the shape, contour and size of your breasts prior to your implant removal, the most reliable, simple and predictable way to return to your previous shape, is to undergo a new breast augmentation with implants. Your photos show your nipple/areolar complex is in excellent position, and therefore there is no reason to do an additional breast lift. Fat grafting is an option, although I believe a poor one, as an implant will give you a predictable volume and shape for years to come and fat grafting will lose some volume after the procedure and can result in irregular breast contours where non-viable fat is resorbed and requires another procedure to obtain the fat used in grafting. You already know how you looked with 350 ml. implants. A question now is how bigger or smaller you want to be; or 350 is your size? Discuss this issue with your surgeon ahead of time. Good luck.
