While there is some blood loss during this procedure, it should not be anywhere near needing a transfusion. We have performed hundreds of "genios" and never once needed to transfuse anyone with blood. That being said, we specialize in this procedure and are able to complete the surgery in about 45 min - 1 hour. For surgeons who do not perform this surgery as frequently, it will take them more time, thus extending the time a person will be bleeding. Still, there should be no need for a transfusion if the surgery is done correctly. If you have found a surgeon who says a transfusion is necessary, I would look elsewhere for another consult.

Based on your pictures, I think that you would benefit from a 3D/OOS lip lift, a lower V-Y and maybe a corner lip lift as well as a facial liposuccion and a rhinoplasty. About the chin, I would do Botox first (to relax the mentalis muscle) as it offers really good results in that situation.
First of all, you are obviously an attractive girl. The easiest (always start lighter to see if you like it) would be Botox under the nose but it is not permanent. A Frenulectomy (or frenectomy) can be performed with or without weakening of the muscle.
