I am 4 years post-op breast lift with Natrelle SRM 360cc implants. I want a breast revision to give me an augmented look. Although my nipple placement is good, my breasts still sag. I have been to 2 consults for this (not my original surgeon). Both surgeons suggested a revised lift, highly cohesive implants - one suggested 520cc Natrelle SCF, & Galaflex placement. I am 5’6, 140lbs. 32 years old. Would love to hear your thoughts and possibly meet with you!
Answer: Revision surgery Dear LHudson92, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. Answer: Breasts Your breasts, especially the lower part, are sagging and you need a lift. I do not suggest going larger for your tissue will not support the weight. With your anatomy, you will not have high round implants. Your breasts naturally sit a little low on your chest and the bases cannot be moved.

Answer: Breast revision Thank you for your question. A large portion of my practice is dedicated to breast revision surgery. I have developed a surgery that is best suited for your desires. Is called reductive augmentation. I published this in 2017. You do not need any meshes or additional foreign objects in your breast. The surgery includes an implant exchange ultrahigh profile silicone implant, internal bra-Capsulorrhaphy, and revision of your lift-mastopexy. You should be able have an excellent result. Best wishes to you.

Answer: Revision breast surgery You will need a larger implant (TBT, but probably nearly double), a capsulorraphy with mesh, and a revision breast lift.

Answer: Explant with Bellesoma Method At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
