It would help to have a photo also while standing, but your issue of implant lateralization is a common one. It should be treatable with repairing/tightening your implant capsules on the sides and/or placing slightly wider implants. In-person examination would be required in order to devise the plan that would best address your concerns and meet your goals.

many women seek improved cleavage. Narrowing the cleavage plane can be done very carefully by a skilled surgeon. Moving the implants too far medially can result in synmastia or unaboob which is very difficult to correct. That being said, the right amount of pocket dissection and implant selection can create excellent cleavage. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

I recommend exchanging your implants for smaller implants and doing lateral plications to keep the implants from falling laterally. At the same time, you need the nipple-areola complexes repositioned through a circumareola approach.

The answer to your question is YES. One must first understand why the implants are lateralizing when you lay supine. Commonly, this results from smooth surface implants in the sub-muscular position. The inclination of your ribs, if laterally canted, may contribute or exacerbate the problem. Width of your implants relative to implant pocket size, nipple position, and surgical positioning of your implants also may play a role. If you have a revision procedure to medialize your implants, lateral reinforcement in the operating room should be part of the plan, a post surgical bra that holds implants toward the midline, and if the implants are sub-pectoral (beneath the muscle), textured surface devices so they stay toward the midline and skillfully accomplished medial pectoralis myotomies will help.

You did not post your initial standing photo nor your current standing photo, but I suspect you have a wide space naturally between your breasts. The bases of your breasts cannot be moved closer together. However, your surgeon could tighten the lateral pockets so that your implants do not fall as much to the sides when you lay down.

In complex cases like yours it may be best to remove the implants ,let everything heal and then re assess

To make a quality assessment, we need quality information.
That generally means a complete set of standard before and after pictures.
If you don’t have your before and after pictures, then ask your surgeon to forward the pictures they took.
I’m guessing your breast sit wide on your chest wall.
Plastic surgeons can’t move where your breast sit on the chest wall.
If the implants are placed towards the midline, but your breast sit towards the side, then the implant won’t be lined up correctly on the implant and your nipple will sit off to the side. I recommend you rely exclusively on in person consultations.
