It appears that  some of your nipple is still  alive. However, the surrounding areola may be compromised to a slightly greater degree. Appropriate antibiotics may be of some value. Overtime you may see a surprising improvement  of the final result since it appears that some of the nipple tissue is still viable.

Dear JenWagy,Based on your photos it does appear that you are experiencing some necrosis but a good portion of the nipple and areola appear viable. There will be a process of the dead parts sloughing off and fresh and more bloody looking tissue regrowing. Keeping the area moist will promote healing, and frequent follow-ups with the office are important to monitor your progress as your wound care needs may change. Hopefully you are not a smoker and are avoiding any second hand smoke because these are the kinds of healing problems we frequently see with smokers. Continuing to smoke can make things worse.

Hello dear, thanks for your question and provided information as well... I have to say that I'm very sorry for your concern and result.. It seems that you had developed a nipple necrosis after your surgery, please get in contact with your surgeon as soon as possible, to prevent an infection, hugs!

Based on your photographs,  I think that in the long run you will be just fine.  It seems like you have partial thickness loss of areola tissue; this should go on to feel over the course of the next few weeks.  Long-term pigment change may result. Best wishes.
