A second rasp procedure can be undertaken to soften the residual dorsal hump you have.  However, you may consider a revision to reduce both your cartilage and bony dorsum slightly further for a nicer overall result.

Over many years and no skin change after rhinoplasty and sometimes the nasal passages can become narrow.  He need to be examined by an otolaryngologist for evaluating the status of his nasal septum, nasal valve and turbinates.RegardsDr. J
