I had a sliding genioplasty over a year ago with iliac bone (ouch + scar). However, we discussed a medium movement, and now my chin seems long and unproportional. What are my further options here? I also have lower lip incompetence. Probably because the tissue/muscle can’t accommodate the stretch. Original surgeon (plastic surgeon) says he sees no issue.

Without getting involved with extremely complex surgery I think your face is fairly well balanced and you should leave good enough alone.

Your chin projection looks good, however, the length of your chin appears short and therefore gives you a deep mentolabial fold and and rounded appearance. I would recommend a CT scan to determine your chin height. A vertical lengthening genioplasty will likely give you the look that you want.
From your pictures, it looks like your chin has fairly good projection, and advancing your chin while it might help balance, your face will create a deep labio mental fold.I’m guessing the lack of mandibular projection is actually a little further back in the body of your mandible.
