15 years ago I underwent a full lift and 350cc implants placed under the muscle (this was after a 110 lb weight loss). Fast forward to present, I have lost fullness in my upper pole area and my pec muscles overtime have pressed my implants down and out. My surgeon suggest that I go with 600cc silicone implants (this was based off of the width of my breast) and to be placed over the muscle. Thoughts? 600cc sounds huge!

Answer: Bigger implants over the muscle To most accurately answer your question would require an examination and additional information. Of particular importance would be knowing if you generally satisfied with your overall size, or wanting to be bigger. From your photos, you have developed some recurrent ptosis over time . Revising your mastopexy alone should be able to correct the shape and provide a nice cosmetic result. If you are desiring to be significantly larger, you could exchange the implants at the same time, although In my opinion 650cc seems too large. I also would definitely not recommend repositioning implants over the muscle, as this interrupts a great deal of the blood supply to the breast, increases the incidence of capsule contracture and adds no significant benefit.

Answer: My surgeon suggest that I go with 600cc silicone implants and to be placed over the muscle. Thoughts? (photo) It's all about proportions. Tall women with larger frames and broader chest, shoulders, and hips can often handle larger implants. I would recommend that you go with the size that boosts your self-esteem, and fits your anatomy. Choosing an implant based on an aesthetic goal that exceeds your anatomic capacity may lead to complications and possibly revision. Any added weight to the breast will have a greater tendency over time to increase drooping; placing them under the muscle will help.
