What criteria does Blue Cross Blue Shield of Georgia have for breast reduction coverage?

I am looking to get a breast reduction covered by blue cross blue shield georgia. I saw a question similar asked a couple years back about get BCBS to cover breast reductions and I wanted to see if things have changed/ get a more specific answer. Do they go only based on bmi/ amount removed or does there need to be a record of discomfort with a doctor as well?
