In my opinion, a tummy tuck may eventually be helpful if you have loose skin, stretch-marked lower abdominal skin, or muscle separation, but it is usually best done when your weight is closer to a stable goal. At 5'6" and 220 lb, further weight loss could change the amount of loose skin and the shape of your abdomen, and surgery at a higher weight can carry more risk and less predictable contouring. A tummy tuck is a body-contouring operation, not a weight-loss procedure. If most of the fullness is fat and your skin still has good elasticity, weight loss first and possibly liposuction later may be considered. If the skin remains loose after weight loss, a full tummy tuck is often more effective than liposuction alone. I would focus first on reaching a healthy, stable weight, then have an in-person exam to decide whether tummy tuck, liposuction, or a combination is most appropriate.
