Hi, I had butt implants done in 2017. Unfortunately it went downhill from the start. I had 360cc smooth implants put in in 2017, they flipped on both sides but 1 side particularly flipped multiple times a day .In 2020 I had my pocket tightened and put in the same implant size and shape but it didn’t work and the pocket loosened again soon. In 2023 I replaced my 360cc implants who were also very saggy to 560cc biconvex and again tightened the pocket. This also did not work and the pocket loosened

Allow me to answer your question as best i can based upon your photo. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

Based purely upon only one photo and your history, along with my 23 years experience specializing in buttock implants and revision of buttock implants, i am going to assume that the main problem is that your implants are not completely 100% within the muscle. Also, I am not a big fan of the “biconvex” round silicone gel implants because they are not anatomically correct for the buttock/gluteus muscles.

If my assumptions are correct, then there is hope for you. The implants need to be removed and replaced to a pocket that is 100% within the muscles. Also an implant exchange to a more true anatomical shape implant such as Stanton Anatomic®️ buttock implants will likely serve you best.

Please note that an extremely few number of board certified plastic surgeons specialize in buttock or hip implants, so you may need to travel far and away from home to find this particular surgeon.  Glad to help.

Hi,Yes, you should removed your implants. They will continue to drop and stretch the pocket over time. This may eventually need skin removal or a buttock lift if the soft tissue stretches out. You can opt to volumize or correct any contour irregularities with a fat transfer to the subcutaneous tissue (NOT THE MUSCLE).Hope this helps,Dr. Dev
