Can Gynecomastia crater revision be done with fat grafting? Fat flap? Pec implants? (Photos)

I would personally advice a combination of revision liposuction around the crater and fat injection to fill the sunken area.

Unfortunately fat transfer can take more than one session to be successful .

Thank you for sharing your question and photographs and I am sorry that your results have not been what you anticipated.  Depending on your desired goals I believe that all of your listed procedures are possible to create an improved chest contour.  If you are looking to augment the size of your chest, then a customized pec implant would afford you the best option, while fat grafting/fat flap will allow you to smooth the current appearance to leave less of an indented appearance.  Hope this helps.

Based on your photos I personally would consider a more thorough treatment of your chest particularly in the Zones of your chest that still has fat. By making the layer of tissue more even this will improve the appearance of your chest significantly and it is a reliable procedure with little downside.

Revision surgery for a crater might do well with fat injection. ADM's might be an option as well.  Best of luck.

The crater is not visualized well in the pictures.You need a personal examination. There is a lot of fat in the lateral chest. Liposuction of that area may improve the appearance. some fat grafting may give more enhancement, Implant can only be considered if you need more pec enahancemnt

Thank you for the question and the photos and you have a lot of fat laterally which is sculpted would give you an overall better result  and fat or dermal fat grafting could be done at the same time to the areola area.  An examination though is really need to see what might give you the best result.Dr. Corbin
