Hello @Nkas954 thank you for your question, there are several reasons for why the form is different than your goal look. What I can see here is that the placement of the prosthesis is too high and is behind the muscle. The solution is a breast reconstruction to make the breast look according to my criteria with an adequate aesthetic result. That is, a breast that has a proper shape and proportion, a little conical and that has a little descent in the upper part. Best wishes!

basically the lift wasn't aggressive enough- and your implants are stuck up high and your tissue is sagging off of them like they are melting. Another lift can give you a better shot at good results!

The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

In the photo, you have a "waterfall deformity" which is a very high sitting implant (possibly from capsular contracture, or the aging process, or a combination). This will require a lift and an implant carefully selected to fir the new amount of skin and tissue left after the lift.

Over-augmented breasts , anchor-scar breast techniques, and capsular contracture are the most common causes for a boxy, "pancakey" or square-shaped breast.

Your main problem is that your nipples weren't lifted far enough. This is not uncommon. Your revision should concentrate on moving your nipples to the right place. That alone will improve the appearance.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

The square shape is from your own breast tissue which should be corrected with the lift. Your higher implant can also be lowered. Please do not go bigger for you will sag again.
