Super dented right butt cheek due to trauma and undergone surgery. What can be done to fix this problem? (Photo)

Question

When I was between 5-7 years old a doctor performed a surgery on my right cheek due to an accident/trauma on my right cheek and became cystic. My right butt cheek feels like there is no fat or muscle underneath just hollow with tight skin where my stitches from surgery was. What are my options to get this fix and greatly improved my right cheek so that is it even to my right? I'm considering a BBL, but I am very tiny person.. I'm now on my 30's, 5'3" tall and weigh 100lbs

In general your options are fat grafting (aka BBL) or implant to fill in your right buttock cheek dent. Although typically I am more of an implant proponent for typical cosmetic buttock augmentation, I do not believe even a custom implant will produce a natural harmonious surface contour in your case.On the other hand, you may be a very good candidate for a trial of fat grafting combined with stem cells - Understand that stem cell therapy is still experimental in the commercial setting and results vary. I would recommend excising the scar tissue and injecting fat into the area.  It may require a second fat grafting session but you should see noticeable improvement.

It is a little hard to tell from just your photos, but I think you would be a good candidate for fat grafting to the deficient area. You are very thin, but hopefully there is a few small pockets of fat that can be borrowed. If the scar is stuck down to underlying tissues then you may need a scar release also. With difficult scars sometimes it takes more than one grafting procedure to get the best result.

This type of scar can be improved first, by releasing the adhesions to the muscle, and then using a filler. The best filler will be your own fat, but if you are very thing and don't have any fat to spare, Sculptra (injectable poly-L-lactic acid) is a good temporal option.

Hello!It looks like you lost quite a bit of your fat in that area due to the trauma, and so a fat transfer would be the ideal treatment. It might be a good idea to do this in a few stages though to gradually build the area up. If the surgeon puts  too much fat into that area in a single stage, I fear that much of it will not take, and just melt away.Good Luck.

While it may never be perfect and may take several procedures, fat grafting of the defect in your buttocks is the best and safest procedure. Make sure the board certified plastic surgeon has extensive experience with fat grafting and ask to see photos of their work.
