Doc told me to gain about 20 lbs for a Brazilian Butt Lift. What happens if I can't gain it? Will all the fat be lipo out?

I had a consultation with a few doctors for a BBL. Some say its best to gain 20 pounds while others haven't mentioned any weigh gain needed. I've had laser lipo on my abs and waist before. It's hard to gain any fat there. I do have fat in my back and inner thighs. I weigh 140 lbs and I'm 5'5 tomboy figure with muscle. No curves. If I gain 20lbs would it all be suctioned out? I don't want to have fat leftover that I was made to gain and the doctor wouldn't even be able to remove it.
