Doc told me to gain about 20 lbs for a Brazilian Butt Lift. What happens if I can't gain it? Will all the fat be lipo out?

It is difficult to comment without pictures. However, in general, I do not believe in weight gain prior to BBL. If you lose the weight that you have gained after surgery, you would lose all transplanted fat volume as well. You would need to see an expert plastic surgeon who knows how to harvest fat from various areas

Thank you for your question. It is very difficult to respond to
your question without any photos. However, 20 pound gain in your weight is a
bit in excess. One of the major aspect of buttocks augmentation with fat transfer
is to lipo-sculpture around the buttocks to create an attractive frame. The result
of your “Brazilian butt lift” will depend on how much fat you have for transfer
and your expectations. I would recommend you to see another board certified
plastic surgeon for a second opinion. Wish you the best.

It is not typically recommended that patients gain weight for the purpose of a cosmetic procedure, especially a BBL. It is best to be as close to your natural/goal weight before undergoing such a procedure. Thin patients can still achieve their desired results if they find a plastic surgeon who has experience with such cases. 

I never ask my patients to gain weight for a BBL, for 2 reasons. First when you gain weight, you are not increasing the number of fat cells, you are only filling this cells more. the moment you loose the weight, you will also loose it on the graft. Second, How can i guarantee that this weight you gained will be in areas where i will be able to easily suction?? i don't agree in huge liposuction of all the body, Liposuction is for localized fat, not for loosing weight. Trust me you will have left over after your surgery if you gain 20 more pounds.As some of my colleagues mention here, fat transfer is not the only option for Buttocks augmentation, and even if you don't have enough fat for a gigantic increase on volume of your Butt, it will probable be enough for reshaping it, and give you a really nice contour.If you have any questions feel free to ask, we are here to help.Best of luck,Dr. Luis Redondo

An important part of the consultation is to determine the amount of fat available for transfer. The more fat to be harvested , the more fat could be used for the augmentation. Only 30-50% of the lipoaspirate could be used for injection.  Although that weight gain will increase the amount of fat that could be harvested, it is a difficult recommendation to give to a patient. 

Hello, Thanks for your question. I never ever tell my patients to gain weight. It's simply against all medicine principles to get unhealthier in order to look better. Unfortunately, you did not upload photos to comment more objectively on your case. What I can clearly tell you is that, if you do not have enough fat there are is another procedure called Gluteoplasty,  which can help you improve the size, shape and/or projection of your butt by placing silicone butt implants. As we can notice in you question you are already in overweight and is not healthy to recommend you to gain weight or fatty tissue for that purpose. Obesity is not normal, is a metabolic disease. If with the fatty tissue that you have now due to your overweight is not enough to perform the BBL you are a candidate for a Buttocks augmentation with silicone implants.
