While fat grafting to the breast can be done under local, it would be extremely uncomfortable. The anesthesiologist have developed a TIVA anesthesia which is very gentle and there is almost never post op nausea. It would be best for you to have an in office consult with a Board Certified Plastic Surgeon to discuss the procedure and the anesthesia.

When you develop spontaneous Ptosis of your eye lid you may have a medical problem. You should see a Neurologist as soon as possible to rule out carotid artery dissection or a neuromuscular disease.

If you have tight skin but bulges of fat under the skin then liposuction alone is the procedure of choice. This can be done under your chin on your neck, on your upper back and breast and arm, your abdomen, your medial thighs and knees, your lateral thighs and even you lower legs. Any where there is extra fat and the skin is tight is a good place to have liposuction alone. If the skin is loose it will still be loose or looser after liposuction./

Tummy tuck is a combination of surgical procedures which includes removing the extra skin of the abdomen and tightening the rectus muscles which are separated with pregnancy or weight gain. This is usually all the extra skin from just above the pubis to above the umbilicus. It is usually combined with liposuction of any or all of the areas mentioned above.

Like any elective cosmetic procedure there are trades that have to be made. While you have attractive mature breast, you do have some natural sagging which could be dramatically corrected with a mastopexy (breast lift). the scar would be circular around your nipple areolar complex, and down th e front of your breast with or with out a curved incision under your breast. The need for which kind of incisions which would serve you best can only be determined with a consult with a Board Certified Plastic Surgeon. It would also be a good idea to exchange out your saline implants  for Siltex textured gel filled breast implants.

Since you want to have a very full upper pole with a fake look, you will need to have breast implants along with your breast lift. Best thing for you is to seek a in person consult with a Board Certified Plastic Surgeon who has experience with these procedures. Ask to see before and after photos.

From review of your photos, you have a large nose with a wide base and dorsal hump, mid vault collapse and a bulbous tip. These all can be corrected with a Rhinoplasty. Your short chin can and should be addressed with an extended silastic chin implant. You will need to have a consultation with a Board Certified Plastic Surgeon with experience with these procedures. Ask to see photos of their work. You also have so other facial changes that could be addressed but its hard to give you complete recommendations from your photos. An personal evaluation is necessary to get the best advice.

 From your photo your chin is short and you would have a wonderful improvement in facial harmony with an extended silicone chin implant. This is place through a small incision under your chin. Since you don't want a face lift but want some tightening of the loose skin in your neck, you would be a candidate for THERMItight. This is a radio frequency treatment that is done in the office under local anesthesia and involves heating the skin and soft tissue of neck and lower face. It gives a nice improvement but not as complete as a face lift. Another non surgical approach to tightening the face and neck is Ultherapy which uses ultrasonic focused energy to tighten the skin and soft tissues. Go on the internet to find a plastic surgeon who has this technology.

While you do have a slight dorsal hump, you have a relatively attractive nose. What seems to be the most dramatic problem with your profile is your short chin. An extended chin implant would do much to improve your profile and make your nose seem much smaller because of the visual relationship between your nose and chin. You could also have a small dorsal reduction rhinoplasty.

Your photos show that you have lipodystrophy in your hip and abdominal area lose abdominal skin. You are a great candidate for liposuction of your abdomen and hips and abdominal paniculectomy (Tummy Tuck) and plication of your abdominal muscles. You may also have other areas which would be benefited with liposuction but you will need to have an in person consultation with a Board Certified Plastic Surgeon. Good Luck

Your photos show that you are a good candidate for a Rhinoplasty and chin implant. An open technique would be best. You can have the dorsal hump removed, the width narrowed and the bulbous tip refined. Your profile reveals you have a short chin which would do well with an extended chin implant. This will balance your face and even make your nose look smaller.

Normally I pack the nose, tape and splint the nose for a week after surgery. I remove the splint and packing after a week and re tape the nose for another week. During this two week period you need to avoid lifting, bending over or performing activities or exercise that would get your blood pressure and pulse up.It is during this two week that there is a risk of bleeding.  After the two weeks you can exercise but must be careful not to hit or bump your nose for a total of 6-8  weeks. After that you can resume normal activities. 

You should consult with a Board Certified Plastic Surgeon with extensive experience. Ask to see before and after photos of your surgeons own cases.

Dr. Lentz and his staff were truly exceptional! They took care of all of my needs and wants and gave me the best look for my body when it came to implant size and shape. I had a mole growing in my eyebrow. I decided to have it removed and decided to approach three surgeons for opinions. The first two explained that removing the mole would be very tricky and could leave a scar. They quoted me prices in the $2,500 to $3,800 range if I remember correctly. I walked into Dr Lentz's office and he demonstrated his ethical nature and creativity. He immediately said, "I think you will be trading the unsightly mole for a scar. I have a better idea. Let me shave it off. This will not make it go away, but will get you years of no longer noticing it." He also said he could take off three other moles the same way. I asked how much. $600. I was very skeptical and worried to let him do something the other two surgeons had not even mentioned as an option. I had been told Dr. Lentz was very good, so I let him do the procedure which he did that day. Wow. Fantastic result. Six weeks later, you would hardly know the mole was embedded in the eyebrow . The other moles we not only gone, to this day I cannot tell you where they were. The point is Dr. Lentz is NOT one of those people who "only have a hammer and treat everything as a nail". His creative approach solved my problem in a much more effective manner. No worry about a scar, mole no longer an issue. I strongly recommend using Dr. Lentz. I have been using his services for Botox and fillers for years now. I will be using his services for facial plastic surgery in the future. He is very very good.

Being Wrong Was the Best Ever - Daytona Beach, FL

I was the very last of my friends to go “under the knife.” We talked about plastic surgery constantly – what we would do if we had the money. When I finally got around at age 42 to have a breast augmentation I approached it with absolute certainty. I knew what size (300 cc), what type (saline), what procedure (armpit), I wanted. I actually went into Dr. Lentz’s office for my consultation and told him how to do his job. He smiled, took my hand and said, “let’s talk.” We discussed every option, recovery time, scar potential, nerve sensitivity, and size until we were blue in the face. I tried on implant after implant – with a tee-shirt, without a tee-shirt for what seemed like hours on two separate occasions. He was very patient and allowed me the time to come to my own conclusion about what was best for me and my frame. I ended up going bigger than expected (450cc), selected silicone gel vs. saline and opted to use my nipples as the incision sites. Crazy, right? I must say that I have always had a hard time admitting that I was wrong . . . but NOT THIS TIME! I am amazed by my entire experience and my results. It’s been about 4 months since my surgery and I still wake up and say, “I was wrong - and it was the best thing that has ever happened to me.”
