If the first pocket is good, then only the space above the muscle is opened and the rest is left alone. Your surgeon is the best one to tell you what specifically he/she will do.
If your implants are not in the correct plane (i.e.: Too lateral), then yes the pockets should be plicated. Moving the implants from under the muscle to over the muscle does not require suturing of the pocket.Dr. Taranow
