It's hard to tell from your photo exactly what is going on, however it looks like there is some delayed healing.  It's best to ask your doctor's office exactly how to proceed with would care issues in these situations.  Hope this helps...

You need to see your operative surgeon ASAP - like TODAY! Your bb is 100% necrotic and needs aggressive surgical debridement. Please seek in person medical care.

I recommend contacting your surgeon to see how to proceed with wound care.  It seems appropriate to clean the area before applying the cream, but your surgeon should advise you what to use on it.
