Ok so I have lost a total of about 140lbs I'm 5'8 and currently 176lbs, I jUst had a BBl done July17th. I'm going for a tummy tuck probably in Jan. I have had 3 consults two of them say I would just need a extended TT and it would get rid of the area I circled in red, One said I would need a FDL tummy tuck(where they cut you vertically and horizontally)to get rid of the area I circled in red. I need opinions will an extended take care of that area or will I need the FDL?
Answer: Am I a better candidate for FDL tummy tuck or extended TT? Looking at your pictures, you definitely will benefit from an extended type tummy tuck, but it may not get rid of all of the excess skin in the upper abdomen. A fleur de lis will require an incision that goes all the way up to your sternum to adequately address the circled area which in my opinion leaves a scar that is often not a great trade off for the benefit gained. An alternative, would be to do the extended tummy tuck, then do a procedure called a reverse abdominoplasty where any excess upper abdominal skin remaining is pulled upwards with an incision placed in the inframammary folds of the breasts. This hides the scars and can be combined with a breast lift if desired. The reverse abdominoplasty must be done as a separate procedure from the extended tummy tuck to ensure that there is good blood supply to the tissues.

I would recommend you consider an extended Abdominoplasty which can be possibly incorporated with the FDL. The FDL decision is best made during the procedure but consented for pre-operatively. This gives the Surgeon the benefit of all available tools to achieve your maximum result. I would be surprised if the upper abdominal fullness could not be incorporated with the previously mentioned procedures. I don't think a reverse lift would be necessary. But you need an in person exam by an experienced Board Certified PS who specializes in post weight loss reconstructions

Answer: TT The area circled in red would be undermined in a standard tummy tuck incision without needing to perform the FDL procedure. It appears that this area would be corrected without having to resort to the additional midline incision that is part of the FDL.
