I had lower bleph with fat repositioning. It was a mistake and I know know I was not a candidate. The results are very bad and I really just want to move the fat back up to a higher position/ original or close to original position. Is this possible? I don't want fat transfer.
Answer: The answer very much depends precisely what is going on. The transposed fat scars into position at the top of the cheek. In some cases, only part of the lower eyelid fat its transposed and some is cut out. That means that in revising these, it is seldom possible to "reverse" the surgery. In some cases, scaring can also compromise the lower eyelid so that the issue can be loss of lower eyelid volume with some type of post blepharoplasty lower eyelid retraction. I have developed the most advanced system for reconstructing the lower eyelid. I am attaching a surgical video below that details how this is accomplished. Please note that what you might need will very much depend on what resources have been left by your prior surgeon and what resources are missing. However, it is seldom a matter of just "releasing" the transposed fat. There is no substitute for a detailed in person assessment.

Answer: I would recommend endoscopic midface lift. Hi, Disneymom21, thank you for your question and photos. You have hollowness under eyes on both sides. There is a volume deficit in these regions. It is possible to lift the midface and support your malar fat pads with an endoscopic (or classic) midface lift operation. This lifts all this fat tissue that has gone down with time and gravity. So the hollowness under the eyes are reduced significantly.

Answer: Moving lower eyelid fat Yes it is possible - the reposition or to debulk. But it might be that what you really want is a midface lift, to make the transition between the lower eyelid and the cheek smoother.

The problem is not the fat moved from above but lack of volume below. Treatment is addition of volume in the deficient areas using either fat or a filler like Juvederm Voluma.
