68 y/o interested in eyelid surgery - would transcutaneous or transconjunctival blepharoplasty deliver best results? (photos)

Question

My question is what is a better opion for me, doingtranscutaneous or transconjunctioval bletroplasty for the lower lids. I didn’t have transcutaneous surgery 15 years ago. Is redoing the surgery with a cut underneath is advisable for second time. I am 68 years old and at this age my skin is not very flexible anymore.

Hello! Thank you for your question.As far as I can see from your photos you have extra saggy skin both under and above your eyes. Also your mid face area is very downward. With the transconjunctival lower blepharoplasty It is possible to clear extra saggy skin and strecth the mid face. So the transconjunctival lower eyelid blepharoplasty is more suitable for you. Face to face consultation is better way to make this decision. For get rid of the extra saggy skin above your eyes I suggest upper eyelid blepharoplasty. Best Regards!

For significant improvement, hooded eyelids require surgery if there is enough skin. For nonsurgical treatments: Radiofrequency (Thermage or Venus Legacy/Viva), injectable fillers, and a Botox brow lift can help contour the eyebrows and lift the eyelid skin. For over 30 years, we have performed all fat removal through a trans- conjunctival approach with the incision placed on the inside of the lower lid to remove the fat deposits. If and when there's excess skin located on the outside, then a very small incision is placed directly  below the eyelash line and a very conservative small pinch of skin is performed. We close that incision with tissue glue. No stitches. Transcutaneous lower blepharoplasty can create with scleral show and ectopion at your age. The transcutaneous approach weakens the lower eyelid muscle and is not advisable
