Rhinoplasty - Can the upper bridge be moved to center the nose between the eyes better? (Photo)

Thank you for your question. Facial symmetry is elusive and very difficult to obtain. Also, straightening a nose that's crooked is difficult. I think it would be difficult surgically to move your nose in the direction you desire. Please see a board certified plastic surgeon experienced in rhinoplasty. Good luck.

The simple answer is yes, but it needs to be qualified in a more complex manner. First of all, if you look to align the top of the bridge based on the position of the tip, it is likely to be too close to one eye compared to the other. In your case after the correction, the bridge of the nose is too close to the right eye compared to the left.

In general, a combination of moving the top of the bridge and tip in opposing directions is the solution that is more likely to work. Ultrasonic instrumentation is in my opinion very valuable when contemplating changes like the ones you are describing.

Please bear in mind that perfect symmetry is not possible simply because everything about the face is asymmetric, not just the nose.

Based on these photos, major shift seem to be in the cartilaginous part of the nose/septum; most of deformities of this kind are that way. Surgery should involve septum shift which will place septum in the central position and correct current deviation of the nose. Good question. Yes the upper bridge can be straightened a bit. Where it is over on your right it can be moved over to the left by way of high transverse and radix osteotomies. In addition septoplasty and unilateral spreader grafts will help straighten the tip and midvault respectively

The slight tilt to the nose is likely due to skeletal and soft tissue asymmetry. Imagine that your nose is a house and the cheekbones are the foundation of the house. If the foundation is uneven, then all of the structures above that foundation will sit uneven too. looking at some subtle clues, you have a slightly deeper orbit and nasolabial fold. It is quite difficult to achieve perfect symmetry in rhinoplasty and changing nasal bridge structures alone may not accomplish this goal.

Yes, you have a very common condition called facial asymmetry, where one side of the face is slightly different than the other side. You should consult with a seasoned facial plastic surgeon. You should be able to get a significant improvement with better alignment of the nose. It may never be absolutely perfect, but at least worth getting an opinion.

From your photos, I note apparent oblique "reverse S" shaped displacement of the nasal bones and the nasal septum. Without lateral views, I cannot comment upon "droopiness". The limitations of a 2 dimensional photo of a 3 dimensional subject limits my ability to assess inner canthus of eye to nasal base distances.
Slight adjustment of the nasal root is possible during rhinoplasty (several mm.), but the major problem appears to be an old injury to the the nasal septum associated with a displacement of the nasal bones. If there is no history of injury, an uncommon cause of nasal injury is a vaginal birth. The twisting and turning during delivery of the newborn baby's head may result in an undiagnosed fracture of the tiny "button nose".
Obviously one cannot base a treatment modality based on several small photographs. I recommend a visit to a board certified plastic surgeon for evaluation and discussion. On occasion, cephalometric radiographic studies or computer imaging are required to determine the best modality to achieve optimal symmetry.
