Rhinoplasty and filler - can existing filler affect rhinoplasty results?

Question

So I’m getting a rhinoplasty next month, and I’ve gotten a bit of filler in my nose in the past. I got it to fill in the top of my nose so my nose wouldn’t look like I had hump, some of it has gone down by now but my nose doesn’t look as bad as it did without any filler. So I’m guessing their might still be some remaining filler. My question is will the remaining bit of filler I have left affect the results of my rhinoplasty ? when he opens my nose will he see it and be able to just remove it ?

Hi @giselle_____! This is an excellent question. HA dermal fillers can be dissolved with Hylenex prior to surgery. However, if there is some filler remaining,  the surgeon can just move it while in surgery. I hope this was helpful.

Thank you for your question. We prefer dermal fillers to dissolve completely but ıf you still have some fillers during the surgery we do remove them. It will not effect your results. Best wishes.

Thank you for your question. It is possible for hyaluronic acid fillers to be dissolved prior to your rhinoplasty procedure. The results from fillers generally last no longer than one year after being administered, though the timeline can vary from patient to patient.
It may be best to wait until the fillers have worn off completely or consider having them dissolved so that your nose and other facial features can be accurately examined prior to surgery. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon who can take your timeline and future cosmetic goals into account.

Most of the filler material can be seen and removed during open rhinoplasty. However, in some cases filler may be encircled with a scar tissue, and it may require more aggressive removal. This needs to be discussed with your surgeon ahead of surgery. Good luck.

How much filler of which type, and how long ago? Without seeing any photos or examining you, filler in the nose can last a really long time--I've seen 5-10 year old filler in a nose during a rhinoplasty (for an HA filler that is only supposed to last a year). Best to dissolve as much as you can well before the surgery, to get the truest shape possible. Some of it can be excised during surgery, usually. Some of it may not be able to be dissolved or excised, and that is OK too just something your rhinoplasty surgeon will have to work around during surgery.

Which filler? When in the past? How much? Anyways, the answer to the question you are circuitously asking i.e. "Should I get this filler dissolved preop", is YES, preferably a good few months before surgery.

This is a great question! The answer is yes - having filler placed under the skin of the nose may affect your surgeon's ability to judge the exact shape and contour of the nasal skin, which can be very thin and very delicate along certain portions of the nasal bridge. Depending on your skin thickness, a surgeon may feel that using a medicine to reverse the filler (if it is a filler that can be reversed) is helpful to ensure the best possible results. In my practice, I ask my patients to see me about 1 week prior to surgery, and my preference is to feel the skin, and dissolve any filler placed in the last 12 months. Filler can be visible under the skin, but not always, depending on the depth of where it was injected. I would relay your questions to your surgeon and how this may affect the kind of rhinoplasty you are planning to have. Good luck!

Good Afternoon, Giselle! During rhinoplasty, if filler is still present, it can be removed. Having it all gone prior to surgery will help to determine exactly what deficits need to be corrected. You can consider having the filler dissolved so the surgeon can see your unrefined nose. As for it affecting the outcome, it doesn't always. I have seen cases, though, where filler can cause scarring or compromise of healing in the skin. It's important to let your surgeon know you've had filler so the planning can be accurate. Will pre-existing filler affect the results of a rhinoplasty? it probably won’t have a big effect and depending on the technique used it may be visible depending on what level the filler was injected .some of it may not be visible in which case it will make the root of the nose look slightly higher than it really is which may prompt the surgeon to rasp it lower than he otherwise would. in any event it is easy to remove filler by injecting an enzyme. It takes 48 hours to dissolve once the enzyme is injected. Anyone who does filler injections will be able to do the enzyme injection. So you could either return to your own injector or perhaps your upcoming surgeon will . But I always like to remove any existing filler so I see exactly what I’m dealing with.
