I’m 26, 5 foot 4 and weigh around 10.5 stone. I had a mastopexy with fat transfer last year, however I had to have revision this year as the nipples were too high & too large- they expanded with the fat fill. My surgeon said he would tighten the underneath to position them better but as you can see my nipples/ areolas peep out of my tops or anything below an average length necklace so it didn’t work. I’m disappointed & like I said the 2nd time it has happened now. What should I do?
Answer: Nipples Your nipples are probably where they should be, but you have too much heavy breast tissue below which is sagging down and giving the appearance of your nipples being too high. This is corrected with the underneath incision to remove excess skin and breast tissue.

Answer: Nipple position From your photo it appears the injected fat has settled at the bottom portion of the breast, which has affected the nipple position. There is little upper pole fullness. I encourage you to meet with a board certified plastic surgeon near you to discuss your options.

Answer: Are my nipples/ areolas too high after a breast lift with fat transfer? Hello @Raeann7, thank you for your question and showing your picture. It is possible to reposition and improve the shape of the breast with minimal scarring by raising the base slightly and improving the overall proportion. This will create overall harmony. For more information and recommendations is best to consult with one or several board certified plastic surgeons. Best wishes! Alan Gonzalez MD, FACS.

Answer: Revision surgery Dear Raeann7, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.
