Implant sizing - is 350cc the largest size I should use? (Photo)

Question

I currently have 550cc ho over the muscle implants i had 10 years ago which have now dropped and displaced due to weight loss, there is also a lot of riplling u can see and feel, my consultant haa suggested a maxtopexy with new implants under the muscle in a new pocket I am happy for all of this apart from the sizing as ge said he can only use a maximum of 350cc and I was hoping for around 425cc

Your biggest problem is your implants are at one level and your breast tissue is at another. That is why you have the contour deformity. Your breasts are very large and wide on the top and very low on the bottom. At this time, I recommend explantation and lift using the Bellesoma. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

Hi eststeadFrom your photo, there appears to be significant implant descent (bottoming out) with visible rippling and thin tissue coverage, likely worsened by weight loss. In this situation, your surgeon’s recommendation to downsize and move the implant under the muscle with a lift is aimed at improving support, coverage, and long-term stability.
Going from 550cc to 350cc is not just about size&mdash;it reduces:
Weight and downward pressure on already stretched tissueRisk of recurrent bottoming outVisibility of rippling, especially with better muscle coverage
Trying to place a larger implant like 425cc in thinner, weakened tissue may compromise the result again, even with a lift and new pocket. The goal here is not just fullness, but durability and shape control.
This is a great question. I would recommend consulting with a board-certified plastic surgeon to further discuss your goals and evaluate you in person.

An in person evaluation is needed to determine the range of implant sizes that would be recommended for you.  If you are not comfortable with your surgeon's suggestion, visit with a few other board certified plastic surgeons near you to consider varying opinions.  A breast lift will be recommended regardless.
