I had 450cc high profile mentor silicone implants over the muscle 11 months ago. I was (barely) a 32A pre op, now measuring a 30E or a 32DD. I am not worried about bra sizing etc, but more how my implants look. I love the shape of them in my clothes etc, but they are just too BIG! Now I am thinking of downsizing to 300 or 250cc. By looking at my photos, can anyone tell me if I will need to have a lift aswell as implant exchange and tightening the pocket?

Pocket modifications may be needed in order to properly position an implant and avoid malposition issues, but it is unlikely that much work will need to be done based on your pictures.

Keep in mind as you are thinking about your goals that sometimes adjustment of the breast implant pockets (capsule adjustments) may be necessary to achieve the outcome you are looking for. No breast lifting would be necessary. Generally speaking, the best online advice I can give to ladies who are considering revisionary (downsizing) breast augmentation surgery is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "down a cup size” etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 3. Answer: Downsizing It is not possible to tell whether you would need a mastopexy without being able to assess your tissue laxity in person. Going from 450 cc to 250 cc is a significant decrease in volume. You might do well by having a discussion with your surgeon and giving them some free reign in the ultimate size decision. Sometimes we use sizers which are temporary implants to determine the optimal implant volume without opening extra implants. The sizers cost much less and therefore save you considerable money. If avoiding a mastopexy is a major concern, then you may have to accept a slightly larger implant volume.
