An endotine fixation system is used in combination with an endoscopic brow lift. This option actually raises your hairline. To lower your hairline, often called forehead reduction, you will need to make a "trichophytic" incision at the hairline, release the scalp and advance it downward, and then excise the now "redundant" non hair-bearing skin of the forehead. This is often combined with a trichophytic brow lift. The endotine fixation system is not required in this type of surgery.
You can advance your hairline and do a brow lift. I would recommend a more permanent fixation of the brow. You will need a hairline incision anyway so may as well do a formal brow lift. You can resect some of the FH musculature so you don't need botox in the near future. If you are looking for more peak in your brow, you could do a formal pexy to create more definition.
You can have a browlift, with an endotine or similar system to hold the hairline in place, with a scalp advancement. The timing with botox is not relevant. You'll need botox either way to keep wrinkles from muscle movement away. But, the surgery can happen at any time.
If you have in mind reduction of forehead height, it is better to consider open brow lift. Incision is placed in a zig-zag menner along the hairline and is practically invisible. Lowering the hairline with Endotine may affect position of brows. Hope this helps.
Irregular Trichophytic Forehead Lift with Hairline Lowering will raise brows and Lower Hairline. If properly done, it will not leave a noticeable scar. You wont need an Endotine.
Thank you smooch for your question! Based on your pictures you are a good candidate for forehead reduction and eyebrow lifting. However, I will not be considering forehead lifting in your case. You can perfectly benefit from botox sessions for now. Hope it helped!

I do a log of cranial surgery. Adjusting the back of the head can be done with a bone cement, or, with an implant that is a synthetic biocompatible material. A CT scan is usually needed in advance.
