Fat transfer has to have the proper recipient site. It does not appear that you have much breast tissue in the top half of the breasts therefore, not much fat transfer will survive in the top half. You will need a breast lift and I recommend The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Fat grafting is an option based on your expectation. Better option is lift and fat grafting without implants.
Based on your one front of you posted photo that transferred to upper polls of your breast can be done, but it would not affect any type of lifting. Breast fat transfer can increase breast size by about 1/2 cup.It does not change the shape of your breast in any significant way.The procedure is not good at restoring volume in the upper half of the breast.The best procedure to increase volume or projection in the upper half of the breast is augmentation with implants.In your case, you would probably need a breast lift with implant augmentation.If you just want a bigger version of what you currently have than fat transfer can do that.Best,Mats Hagstrom MD
