You can go ahead and get implants alone, but you need to keep in mind that your breasts will only continue to sag more with time which will eventually warrant a lift. I agree you do not need an augmentation. This size of implants would give you nice fullness with minimal scars. Dissection over the muscle can help ensure proper nipple position as well.

Based on your photos, you have some sagging. If you have decided against a lift, then in terms of implant sizing, there are many variables in recommending implant size/type. I base my implant recommendation on patients' body (chest height, chest width, existing breast tissue) as well as their desired look. I usually do not talk about cup size. As you know, cup size varies with brand and styles of bra. I would go for the desired look. Review before and after photos and see what kind of look that you want. Implants come in different types (silicone, saline), shape (round, anatomic), and profiles and volume. I think you would do well with just an implant. The absolute size is a little harder to determine without an in person examination. Most of us will use some measurements of your chest to determine implant sizing. You are petite so a narrow, higher projecting implant is appropriate for you. You've chosen correctly with a high profile implant. I would suggest looking into Sientra high profile implants as they are slightly over filled and less likely to show rippling or wrinkling. That's important when you are thin and I have very little breast tissue to cover the implant. Good luck!
More than likely implants alone would give you a nice volume and shape. Final implant volume sometimes is determined during surgery. Skin quality and tone also plays an important role in the final selection of the implant.
Thank you for your question and photos. A breast augmentation alone under the muscle with a 325 cc implants would give you a nice fuller and natural result. You do not need a lift at this point. Your nipple position appears to be normal. Best wishes!
A breast augmentation with implants will add volume. They do not life the breasts to any significnat degrees. Best of luck.
Your nipple appear to be just below the crease behind the breast but you have at least 3 cm of breast tissue below the crease. This is termed glandular ptosis. I would recommend a shaped anatomic gel implant with enough projection to force the lower pole up. You will not get this effect with a round smooth walled implant and run the risk of having breast tissue that falls off of the end of the implant. I would also recommend placing the implant in the subfascial plane and not the submuscular plane. This will allow for greater filling of the breast envelop. An implant such Mentor MM+ or MF in the 350 cc range would do well or could consider a Sientra round based implant in the same volume.
