I had 2 hypertrophic scars biopsied on my chest and shoulder back in september. After the biopsies the hypertrophic scars came back just as big as before. The dermatologist offered to inject kenalog into them to flatten the raised scarring. This was around the 1st of December. Flash forward 6 months later to the middle of May, the depressions are still deep and a redish purple color. What would be my best option to treat this? Would saline injections and vbeam laser work?
Answer: Hypertrophic scars It's unfortunate that the hypertrophic scars have not responded well to the Kenalog injections, and it's understandable that you would like to explore other treatment options. Saline injections and Vbeam laser are two possible treatments that may be effective in reducing the appearance of the scars. Saline injections involve injecting a sterile saltwater solution into the scar tissue to help break up the collagen fibers and promote healing. This can help to smooth out the raised areas of the scars and reduce their overall appearance. The treatment is typically done over several sessions, spaced several weeks apart. Vbeam laser is a type of laser treatment that targets the blood vessels in the scar tissue, causing them to collapse and reducing the redness and discoloration of the scars. It can also help to stimulate collagen production, which can help to improve the texture and appearance of the scars. Multiple sessions may be needed to achieve optimal results. It's important to note that the effectiveness of these treatments can vary depending on the severity and type of scars, as well as individual factors such as skin type and overall health. It's recommended to consult with a board-certified dermatologist or plastic surgeon who has experience in treating hypertrophic scars to determine the best course of treatment for your specific case. They can evaluate your scars and recommend the most appropriate treatment plan, which may include a combination of different approaches. Best of luck !

Answer: Recommend doing PRP and adding a small amount of filler to minimize appearance of indented scar Our treatments use combination therapy to improve the epidermis and dermis for most deep scars. The options available for acne scars depend on the character of your scars, in terms of topography and contour. We see atrophic and hypertrophic acne scars, as well as scars that have hypo and hyperpigmentation. We begin each scar evaluation with an assessment of scar quality and subtype. Our main concern is rebuilding collagen and that is why we use a HIDEF protocol that includes combination therapy. Morpheus 8, scar subcision, fibrous release, fractional laser, CO2 laser, Thulium laser, PDL Vbeam laser, and fractional resurfacing can be used in addition to TCA Cross and chemical peels to further improve skin. Patients should start Melarase AM and Melarase PM for active hyperpigmentation. Always begin with a consultation to decide whether early subcision should be started.
