How to get a bigger butt and hips when petite? HELP!!

 Regarding buttock/hip augmentation surgery, let me start off by saying that I currently see at least 2 - 3 patients, domestic and international,  per week that failed "brazilian buttock lift" (i.e. the fat melted away after 10 - 12+ months and/or the projection result was grossly insufficient) and are now seeking a more reliable and permanent option (i.e. buttock and/or hip implants). Frankly, i have less than a 5% overall complication rate with only 1% infections.Because at least 80+ % of the fat transferred will melt away within a year, most patients are not good candidates because they lack an adequate amount of fat to harvest. Another tip is that if you purposely gain weight (i.e. fat) for the procedure, the fat you lose first as you get back to your baseline weight is that very same fat that was transferred into your buttock...so don't fall victim to this recommendation. Finally, I have unfortunately had many patients come to me after having their high hopes for buttock augmentation crushed by measly Sculptra results. Critical points of awareness are: 1) Sculptra is temporary, it will last 1.5 - 2 years at most, 2) if done correctly dermal fillers like Sculptra are cost prohibitive for 99% of potential buttock augmentation patients. In order to create a significant/noticeable result with a dermal filler, at least 200 mls of product must be injected into each buttock cheek for a total of 400 mls. At $1000 per ml, this equates to a minimum price of $400,000.00. Thus if your doctor is recommending to put in ONLY 20 or 40 mls of sculpture you will pay $20,000-$40,000 and get an insignificant miniscule result!, 3) Do not let anybody tell you that the product is so special that it's going to magically grow over the following months or years...this just doesnt happen to any noticeable degree!, 4) dermal fillers, when injected in large quantities, have a relatively high infection rate, guaranteed tendency to migrate away from the original area they were injected into, and almost always stimulate a lot of inflammation with a subsequent and disastrous amount of scar tissue/hardening, and 5) the one very serious complication that can rarely (< 1%) happen is "embolism" in which some of the filler gets into the blood stream and travels up into the lungs, heart, and/or brain causing serious problems, including death. Hope I at least saved you some money and maybe even your life. PS  Like any booming fad or fashion, there are always scammers looking to get a peice of the action and make quick/easy profits.  Well as the saying goes, "anything too good to be true, usually is (not true)". Unfortunately the ripe new world of buttock augmentaion has become the next victim with a bunch of nonsense magical creams, potions, lotions and pills...NONE OF WHICH WORK. It is not yet possible to take a substance that only and exclusively make your derrière grow. Other than being genetically blessed in the buttock and/or consistent rigorous exercise, there is no "natural" way to grow the buttock other than surgery.

It is very difficult to determine the exact size and shape implant you will require to best match your ideal butt image without an examination by a board certified plastic surgeon. Not just any board certified plastic surgeon, but one with many years of frequently performing breast augmentation surgery including different approaches, techniques and implant choices. This is because several measurements not to mention your breast characteristics are needed to determine the optimal implant size to obtain your goals. Without knowing these dimensions it would be difficult to make this determination. For example, the existing base width of your buttock will determine, in many cases, the maximal volume per implant profile that you can accommodate. Liposuction of your lower back and fat transfer is also helpful as an adjunct to implants. Using “want to be” photos however are useful if simply provided to the surgeon.

Thank you for your question. It's difficult to give exact recommendations without photos or more information. Each patient is different, but generally speaking, a surgeon can find fat in the abdomen, flanks, or thighs to transfer to the bum. You need to be at a stable weight for fat transfer to be effective. If you are too thin for fat transfer, you might consider gluteal implants. This procedure can look very natural, but you'll need to work with a board certified plastic surgeon for best results. 

Dear Remera, Thank you for your pictures.  In my opinion, I would recommend  buttocks augmentation with implants.  I do not think you can get the outcome like Kylie Jenner but you can certainly get remarkable improvement.

     This will provide you the most stable result in the long run. For over 10 years I had practice buttocks augmentation using fat grafts. Although you can achieve good volume of the buttocks with fat grafting IT IS EXTREMELY DIFFICULT TO ACHIEVE ADEQUATE LONG-TERM PROJECTION WITH FAT GRAFTING ALONE. The goal of all women is to increase buttocks projection. The fat is simply too soft to push the firmer tissues of the buttocks skin out to get the desired projection. The best and most consistent way to get projection is to the use buttocks implants.

    The buttocks implants are made of soft solid medical grade silicone. These implants come in various shapes (oval, round), sizes and projections. They are not like the first generation of buttocks implants which were very firm. These soft solid implants that closely mimic and feel like buttocks tissue. Buttocks implants are different than silicone breast implants. Silicone breast implants are made from medical grade silicone which is extremely soft. Silicone breast implants have a small potential for rupture.  The buttocks implants do not rupture or deflate.

    The procedure is performed as an outpatient procedure. General anesthetic is required. The procedure typically consists of enhancing the buttocks through incisions that are made on either side of the upper buttocks crease. This is a very well hidden incision. This allows for the implant to be placed typically underneath the fascia (covering) of the muscle but the implant may be placed within the substance of the muscle if the patient is extremely thin. Additionally, the liposuction may be beneficial to contour the abdomen and waistline.  This will further enhance the body contour.  If necessary the fat obtained from the liposuction can be used to modestly enhance volume.   Incisions for liposuction are very small typically only requiring one or two stitches for closure. Prior to closure of the incisions for the buttocks implants, drainage tubes are required to reduce the possibility of fluid (seroma) accumulation around the implants. The buttocks implants incision are closed with dissolvable stitches. A compression garment is extremely helpful after surgery to minimize swelling, to reduce the possibility of fluid (seroma) collection. After surgery,  one day of bed rest is required. After day one, the patient is strongly encouraged to get up and walk frequently. Typically the first office visit after surgery is within's 48 to 72 hours.

     Remember if you're considering buttocks augmentation with implants it is always wise to consult with a board certified plastic surgeon that has experience with buttocks enhancement.  Best wishes.

Augmentation of the Buttock and Hips in areas where fat does not exist will typically not be successful wit fat or implants.  Sculptra injection into these areas may provide a significant enhancement.  Two to three sessions lasting less than one hour may produce results lasting two to three years. Many pictures that are posted on the web have been photoshoped. Beware of desiring what is not real.Please consult in person with an experienced Board Certified Plastic Surgeon prior to making treatment decisions.

A buttock implant with contouring of your latter flanks, lower back, and thighs is what would get the desired results. The implant will be placed in the center butt and the fat on the hips. This is likely what Kylie Jenner had done.
