I have very ugly and painful scarring even 14 months after my surgery. How can I improve the situation? (photos)

I understand you are concerned with the appearance of your scars. As your scars heal, it should flatten out and lighten. Loss of pigmentation or hyperpigmentation may also be experienced but will resolve over time. Always follow your surgeon's post operative care instructions when it comes to scar management to avoid any complications like infection and for faster healing and recovery.

Based on your photos, it appears that you likely had a very short nipple to fold distance prior to surgery. When the incision is planned in the natural breast fold in this situation (particularly with an oversized implant), the implant can stretch out the lower pole of the breast tissue and displace the scar and natural breast fold onto the now lower pole of the breast. Essentially, it appears that the incision may have ben planned not low enough and the implant placed was too large. Regardless, this a complex problem that should be evaluated and treated by a very experienced plastic surgeon in complex breast revision surgeries. Your treatment would require scar revision with explantation followed by future implant placement with an appropriate size implant.

You might try micro-needling to improve your scars. I would also wear good supporting bras to avoid letting your implants sag down from the weight.

There are many over the counter creams and silicone strips that you can try to improve the look of your scars. You can consider also laser treatments for the scars or scar revision surgery.

Your scar is still in maturation process. Few things might help with your problem. Massage your scar 3-4 times a day for few minutes everyday. Silicon gel application might help to some extent. If these measures are not helping you, you might need to get steroid injection in your scar.

From your photos, I note red healing scars.
Wound healing is a process which usually takes 12-18 months to be completed (sometimes even longer!). I don't think your healing is complete as redness suggests residual inflammation of healing and may also be responsible for your discomfort. This usually responds well in the short term to administration of oral ibuprofen and bra padding. In the long term, this irritation should subside over time. If the discomfort is severe enough to interfere with your activities of daily living, it may be wise to seek the advice of another board certified plastic surgeon.
One can potentially hasten resolution of redness and lumpiness by continuous application of silicone gel sheeting directly to the healing scars.
