Hit in the nose with a baseball bat in 4th grade which led to the bone at the top left of my nose to pop out a bit. 1st rhinoplasty was summer 2017. The bone still popped out and nose tip was huge. 2nd rhino was summer 2018 with same surgeon. Tip size decreased but there's a knot of cartilage on the left side of the tip that can be moved up and down. It causes a bump on the left and my nostrils to be different sizes. Can the cartilage knot be removed/flattened or is a full surgery needed?

Answer: Revision rhinoplasty... Looking at your photos, your tip cartilages appear to be asymmetrical. Your left side is pulled up giving you more nostril show. A good exam is needed to evaluate the status of the nasal bones - maybe they weren't fully osteotomized. A well performed revision by a rhinoplasty specialist may be able to even out the tip, although it may be difficult to make it look exactly even.

Answer: Revision rhinoplasty needed From the photographs presented, you have multiple issues that need to be addressed with A full revision rhinoplasty surgery. A closed rhinoplasty approach can accomplish narrowing your wide nasal bones and making them more symmetric, adding ha spreader graft to the midportion of your nose on your left side where it is collapsed, placement of a composite graft to the left nostril which has alar retraction, shaving down the small dorsal hump which is present, in addition to reducing the hanging columella. Revision rhinoplasty is very difficult, so choose your rhinoplasty specialist based on extensive experience producing natural results.
