Pending replacement of 22 yr old saline implants - will I need a lift if I replace with smaller and or lower profile? (Photos)

My ob/gun recentky directed me to remove my 22 yr old Mentor saline implants . I am 67 yr of age, in excellent health, with no symptoms. My current implants done in 2003 are about 350cc, by a top doctor. While slightly sagging, my nipples are still above the fold. I use pencil in pics to show my nipples are still above the fold. I have noticed when laying down my breasts fall sideways under armpit area. Over the years I regularly did Body Pump weight classes, which may have aggravated, since implants are submuscular. My original doctor retired, so new doctor also highly regarded. I will replace old with new “gummy” silicone implants.. My question is whether I will need a lift if I only replace with a smaller &/or lower profile. Maybe reduce down to 300cc. I wish no visible scars, so wonder about “crescent“ or “donut” other than the usual crease scar. I definitely want to avoid the vertical scar below the nipples. Still nervous about scars around my nipples if a ”donut“-cut. Since I am fair, will any scars around areola be evident? Thinking of a teardrop or wide low profile, rather than round protruding. I want less protrusion. New doctor mentioned he will use mesh for breast sides. Thank lyou for feedback.
